目标

保存 Runtime 输出的薄弱点候选。

字段

id, userId, jobId, snapshotId, targetType, targetId, knowledgePointId, title, reason, confidence, evidence, status (active/dismissed/converted_to_quiz/converted_to_flashcard/resolved)

验收

可从 Runtime result 落库；必须绑定 knowledgePointId 或 sourceBlockIds；保存 reason/confidence；支持用户 dismiss；支持后续转为题目/卡片生成计划
